Submit glass slides, a pathology report (with demographic and clinical information), and representative blocks (if additional studies are anticipated).

The consultation charge is $250 (the Medicare reimbursement rate for California) plus the costs of any additional studies.

Diagnoses are typically reported by telephone and followed the same day by a faxed written report, unless special stains are required which may cause a day or two delay.

Expert medical review of medical records and histopathology material (including any relevant special techniques used to confirm the diagnosis) in cases involving the head and neck region (otorhinolaryngic) and endocrine organs

My rate is $575 per hour (billed in six-minute increments) against a $2,000 retainer to be paid at the time materials are submitted. This is based on CPT code "99075, Medical Testimony." The retainer is credited toward medical data review (CPT 99080), deposition transcription review, deposition, medical testimony, full court testimony, and complete narrative report with itemized bill.

Court appearances or depositions are $3,600 per day. Depositions or court appearances outside of Los Angeles County will incur additional charges for travel time and travel expenses.
